Requirements for Tagging of ANALYST:

1. Photocopy of Professional Regulation Commission (PRC) ID (Back and Forth)
2. Photocopy of Training from National Reference Library (NRL)
3. Notarized Contract of Employment. (Specific as Analyst)
4. Letter of Request from the owner address to Atty. Nicolas B. Lutero III CESO III, Director IV (kindly indicate the email address of the said Personnel/Analyst) 
5. If the new hired analyst was previously employed from other DTL, resignation letter, receive and approved from the previous DTL should be attached.
6. Drug Test Report from other laboratory
7. Medical Certificate (Government Issued)

Requirements for Tagging of Head of the Laboratory (HOL):

1. Notarized Contract of Employment. (Specific as Head of Laboratory)
2. Photocopy of Training from National Reference Library (NRL) if not Pathologist
3. Photocopy of Professional Regulation Commission (PRC) ID (Back and Forth)
4. Request Letter of change of Head of Lab. (kindly indicate the email address of the said Personnel/HOL)
5. Notarized list of DTL's handled by the HOL
6. Drug Test Report from other laboratory
7. Medical Certificate (Governement Issued)

Requirements for Tagging of Authorized Specimen Collector (ASC) and DTL Encoder:

1. Request Letter signed by the owner or HOL
2. Drug Test Result from other laboratory
3. Curriculum Vitae/Resume including Personal Email Address of the ASC
4. ASC Training Certificate signed by the Head of the Laboratory
5. Notarized Contract of Employment
6. Medical Certificate (Government Issued)
