RENEWAL APPLICATION PROCEDURE
- Go to the IDTOMIS Website and log-in the Owner's Account. Select DTL and DATRC Accreditation Menu and Click RENEW button.
- Complete all the requirements needed on the IDTOMIS Website. (Application form, Documentary Requirements, Personnels and Equipment/s).
- After you have completed all the requirements and SUBMITTED the application, PRINT-OUT the Generated ORDER OF PAYMENT.
- Pay either only by DOH Cashier or PMO (Postal Money Order) : Bldg. 15, San Lazaro Compound, Rizal Avenue, Sta. Cruz, Manila, HFSRB (Health Facilities and Services Regulatory Bureau) Note: please indicate Department of Health only in payee or pay to in Manager's Cheque / PMO
- Wait for the email generated of the owner from IDTOMIS of received payment.
